Description
Zavesca (Miglustat) is an oral medication classified as a glucosylceramide synthase inhibitor. It works by blocking the enzyme glucosylceramide synthase, which plays a role in the production of glycosphingolipids. Glycosphingolipids are fatty substances found in cell membranes that are important for cell communication and function. Zavesca is primarily used to treat mild to moderate type 1 Gaucher disease in adults who are not suitable for enzyme replacement therapy.
Directions
To take Zavesca, follow your healthcare provider's instructions precisely. The standard dosage is one 100-mg capsule taken three times daily at regular intervals. If a patient experiences tremor or diarrhea, the healthcare provider may reduce the dose to 100 mg once or twice daily. Always take Zavesca at the same time each day. If a dose is missed, skip that dose and continue with the next dose at the regular time. Do not double the dose.

Avoid taking or using this medication if any of the following apply:
- You have severe kidney problems
- You are pregnant

Cautions
- Before starting treatment with Zavesca, be sure to inform your healthcare provider about the following:
- Any medical conditions you currently have
- Any allergies
- All medications you are currently taking
- Whether you are pregnant or breastfeeding
- If you're taking Zavesca, it’s important to have regular check-ups for nerve health, as some people develop peripheral neuropathy, which can include symptoms like pain, weakness, numbness, and tingling. If these symptoms occur, re-evaluating the benefits versus risks of continuing Zavesca may be necessary.
- About 30% of people may experience hand tremors when starting Zavesca, typically within the first month. These usually improve on their own but if they don’t, adjusting the dose or stopping the medication might be considered.
- Many people experience diarrhea and weight loss with Zavesca due to its effects on sugar digestion. Managing your diet by reducing sugar intake and possibly using anti-diarrheal medications like loperamide can help. If diarrhea persists despite these changes, it’s important to check for other underlying digestive issues.
- Zavesca can sometimes reduce platelet counts, which are important for blood clotting. Regular blood tests to monitor platelets are recommended, especially if you have Gaucher's disease and are switching from enzyme replacement therapy to Zavesca.
- If you experience diarrhea while on Zavesca, try adjusting your diet to reduce sugars and carbohydrates, which might help manage symptoms. Take Zavesca between meals to potentially reduce gastrointestinal side effects.
Side Effects
Zavesca may cause side effects. The most common ones include weight loss, stomach pain, gas, nausea, vomiting, headaches, back pain, and constipation. Some people may also experience dry mouth, muscle cramps, dizziness, vision problems, and changes in their menstrual cycle. These side effects are usually manageable but can occasionally be bothersome.
Zavesca can cause more serious side effects such as peripheral neuropathy, which is numbness, tingling, pain, or burning in the hands or feet. If you experience these symptoms, it’s important to notify your healthcare provider immediately. Other serious issues include new or worsening hand tremors, severe diarrhea, and significant weight loss. Your healthcare provider might adjust your treatment or diet to help manage these effects.
