Ponstel (Mefenamic Acid) is a nonsteroidal anti-inflammatory drug (NSAID) that belongs to the fenamate class. This medication works by reducing inflammation and pain by blocking the production of certain natural substances in the body. Healthcare providers may prescribe Ponstel to treat mild to moderate pain, including pain from menstrual cramps.
Directions
Take Ponstel orally with a full glass of water. The standard Ponstel dosage in adults and children age 14 and older is 500 mg initially, followed by 250 mg every 6 hours as needed. Typically, the treatment duration shouldn't exceed one week. Avoid lying down for at least 10 minutes after taking the drug. Do not take more than prescribed; follow your doctor's instructions..

Contraindications
Avoid taking or using this medication if any of the following apply:
- Hypersensitivity to Mefenamic Acid
- History of aspirin allergy or aspirin triad
- Active gastrointestinal tract ulcer or inflammation
- Recent coronary artery bypass graft (CABG) surgery
- Severe renal disease
- Late pregnancy (risk of premature closure of ductus arteriosus)
Cautions
- Before starting treatment with Ponstel, be sure to inform your healthcare provider about the following:
- Any medical conditions you currently have
- Any allergies
- All medications you are currently taking
- Whether you are pregnant or breastfeeding
- Ponstel, like other NSAIDs, may increase the risk of heart attack or stroke, which can be fatal. This risk may occur early in treatment and may increase with higher doses or longer use.
- Use the lowest possible dose for the shortest time needed to reduce your risk of heart and blood vessel problems. Stay alert for symptoms like chest pain or shortness of breath, and seek immediate help if they occur.
- Ponstel is not recommended after a heart bypass surgery (CABG) or for people who have recently had a heart attack, due to an increased risk of heart-related complications.
- It can cause serious stomach and intestinal issues such as bleeding, ulcers, and perforation, which can be fatal. These issues can occur at any time without warning, especially in older adults.
- If you have had ulcers or gastrointestinal bleeding, the risks of using Ponstel are higher. To minimize these risks, take the lowest effective dose for the shortest possible duration.
- Liver problems can occur with Ponstel. If you notice symptoms like nausea, fatigue, lethargy, itching, yellowing of the skin or eyes, or flu-like symptoms, stop taking Ponstel and see your doctor.
- Ponstel can cause or worsen high blood pressure. Your blood pressure should be checked regularly.
- It can cause or worsen heart failure. Symptoms like swelling of the feet or shortness of breath may get worse. If you have severe heart failure, Ponstel should be avoided.
- Kidney problems can occur, especially in people with pre-existing kidney disease or in those taking certain medications like ACE inhibitors. Regular kidney function monitoring is recommended.
- Ponstel can increase potassium levels, potentially leading to serious heart problems. Monitor your potassium levels regularly.
- Serious allergic reactions, including anaphylaxis, can occur with Ponstel. If you experience symptoms such as difficulty breathing, chest tightness, or swelling of the face or throat, seek emergency help.
- If you have asthma and are sensitive to aspirin, Ponstel could cause severe bronchospasm, which can be life-threatening.
- Severe skin reactions like exfoliative dermatitis, Stevens-Johnson Syndrome, and toxic epidermal necrolysis can occur. If you develop a rash or skin peeling, stop taking Ponstel and seek immediate medical attention.
- Ponstel can cause a serious condition known as DRESS, with symptoms like fever, rash, and swollen lymph nodes. If you suspect this reaction, stop Ponstel immediately and seek urgent medical care.
- If you're pregnant, avoid using Ponstel especially after 30 weeks of gestation, as it could harm your unborn baby by causing heart or kidney problems.
- It can reduce fertility in women. Consider this if you are planning to become pregnant.
- Anemia and other blood-related issues can occur. If you feel unusually tired or look pale, get your blood checked.
- Interactions with other drugs are possible, especially with aspirin or other NSAIDs, increasing the risk of gastrointestinal bleeding. Always discuss your current medications with your doctor to avoid harmful interactions.
Side Effects
Ponstel, like other NSAIDs, may cause gastrointestinal symptoms such as abdominal pain, nausea, vomiting, heartburn, and diarrhea. Some people may also experience headaches, dizziness, and skin rashes. More rare gastrointestinal issues might include significant bleeding or ulcers. It's important to report any persistent or severe symptoms to your healthcare provider as they may need to adjust your treatment plan.
Serious side effects from Ponstel can be significant and may require immediate medical attention. These include symptoms of heart problems such as chest pain, significant digestive tract issues like gastrointestinal bleeding, and liver problems indicated by yellowing of the skin or eyes. In addition, symptoms like swelling of the face, difficulty breathing, or severe skin reactions need urgent care. Always contact your healthcare provider immediately if you experience any of these severe symptoms or if something does not feel right.
