Contraindications
Lariam should not be taken by patients who are allergic to any of its ingredients.
Lariam should not be used for malaria prevention in patients with active depression, a recent history of depression, generalized anxiety disorder, psychosis, or schizophrenia or other major psychiatric disorders, or with a history of convulsions.
Cautions
- Lariam contains the following warnings:
- Patients with life-threatening, serious, or overwhelming malaria infections caused by P. falciparum should receive an intravenous antimalarial drug. Lariam may be given to complete the course of therapy after the intravenous treatment is complete.
- Halofantrine should not be taken with Lariam or within 15 weeks of the last dose of Lariam. Taking these two medications together can cause a potentially fatal heart arrhythmia that results in prolongation of the QTc interval.
- Ketoconazole should not be taken with Lariam or within 15 weeks of the last dose of Lariam. Taking these two medications together can cause a potentially fatal heart arrhythmia that results in prolongation of the QTc interval.
- Lariam levels in the body can be increased by ketoconazole.
- Anxiety, paranoia, depression, hallucinations, psychotic behavior, and other psychiatric symptoms can be caused by Lariam. These symptoms can continue after Lariam has been discontinued. Sucidal ideation and suicide has occurred; however, a relationship to drug administration has not been confirmed. Patients with active depression or with a recent history of depression, generalized anxiety disorder, psychosis, or schizophrenia or other major psychiatric disorders should not take Lariam for malaria prevention. Caution should be used in patients who have had depression.
- If acute anxiety, depression, restlessness, confusion, or other psychiatric symptoms occur when taking Lariam for malaria prevention, the medication should be stopped and an alternative medication should be started.
- Taking Lariam with quinine or quinidine may result in electrocardiographic abnormalities.
- Taking Lariam with quinine or chloroquine may increase the risk of convulsions.
- Before you start taking Lariam, tell your doctor about:
- All the prescription and over-the-counter medications you take
- Your allergies
- Your current health problems and past medical history
- Your pregnancy or breastfeeding status
- Do not drive or operate machinery until you know how Lariam affects you.
- Caution should be used in patients with liver problems.
- Elevated liver function tests and eye problems may occur with long-term use of Lariam.
- Lariam should only be used in patients with heart disease if the benefits outweigh the risks.
