Description
Kevzara, also known as Sarilumab, is an injectable medication used to treat adults with moderate to severe rheumatoid arthritis (RA). It works as an interleukin-6 (IL-6) receptor antagonist, which means it blocks the IL-6 protein that contributes to the inflammatory process in RA. Classified as a disease-modifying antirheumatic drug (DMARD), Kevzara is typically an option when other treatments have not been effective or well-tolerated. As a part of the treatment strategy for RA, Kevzara can help reduce symptoms, improve physical function, and possibly slow disease progression.
Directions
Kevzara is administered via subcutaneous injection. Patients should take the medication under the guidance of their healthcare provider. The recommended starting dosage is typically 200 mg once every two weeks. Patients may self-inject after receiving training from a healthcare professional. During administration, it's crucial to alternate injection sites, such as the thigh, abdomen, or upper arm, to minimize discomfort. Before injecting, check the liquid in the pre-filled syringe for particles or discoloration.
Ingredients
Sarilumab is the active ingredient in Kevzara. Inactive ingredients include arginine, histidine, and sucrose.
Contraindications
Avoid taking or using Kevzara if you have a known hypersensitivity to Sarilumab or any of the inactive ingredients in the formulation.
Cautions
- Before initiating treatment with Kevzara, ensure you provide your healthcare provider with the following information:
- Existing health conditions
- Known allergies
- A list of all the medications you are currently taking
- Your pregnancy or breastfeeding status
- Kevzara can increase your risk of developing serious infections like pneumonia and cellulitis. It suppresses your immune system, making you more susceptible to bacterial, fungal, and viral infections. Avoid starting Kevzara if you currently have any active infections.
- Before you begin treatment with Kevzara, you’ll be tested for tuberculosis (TB). If you have a history of TB or test positive for latent TB, you may need to undergo treatment for TB before starting Kevzara.
- Regular monitoring for signs of infection is crucial while on Kevzara. If you develop a serious or opportunistic infection, treatment may need to be halted.
- Routine blood tests are essential to check your neutrophil counts, liver enzyme levels, and lipid profiles to ensure Kevzara isn’t causing harmful effects.
- Kevzara has been linked to liver enzyme elevations. If you have liver disease or elevated liver enzymes, discuss the risks with your doctor.
- Since Kevzara can increase lipid levels, monitoring your cholesterol is important. You may need treatment adjustments based on these tests.
- There's no confirmed increased risk of malignancies with Kevzara, but cancer has been reported in clinical studies. Stay vigilant for any new symptoms or changes in health.
- Kevzara may increase the risk of gastrointestinal perforations, particularly in patients with conditions like diverticulitis or those using NSAIDs or corticosteroids.
- Be aware of any signs of hypersensitivity such as rashes, swelling, or difficulty breathing, and seek immediate medical attention if they occur.
- Live vaccines should be avoided during Kevzara treatment due to the increased risk of infection. Ensure all vaccinations are discussed with your healthcare provider before starting Kevzara.
Side Effects
The most common Kevzara side effects include redness and itching at the injection site. It may also lead to upper respiratory infections like colds, urinary tract infections, and nasal symptoms including congestion, sore throat, and runny nose. These effects are usually mild and manageable.
More severe reactions can occur with Kevzara, including serious allergic reactions that necessitate immediate medical attention. Symptoms to watch for include shortness of breath, dizziness, swelling of the lips, tongue, or face, intense stomach pain, chest pain, or vomiting. If any of these symptoms are experienced, it is important to seek urgent medical care. Regular check-ups with your healthcare provider are essential to monitor your health and manage any side effects effectively.
