Description
Inhibace (cilazapril) is a prescription medication used to treat high blood pressure. It's also used with digitalis and/or fluid pills to treat congestive heart failure.
Inhibace is classified as an angiotensin converting enzyme (ACE) inhibitor.

Directions
Inhibace comes in 2.5 mg and 5 mg oral tablets.
Carefully read and follow the instructions that come with your Inhibace prescription. If you have any questions or concerns, talk to your doctor or pharmacist.
High blood pressure
If taking Inhibace alone to treat high blood pressure, the recommended starting dose is 2.5 mg by mouth one time a day. The dose may be adjusted based on response to the medication. Patients should not take more than 10 mg per day.
If taking Inhibace with fluid pills, caution should be used. All fluid pills should be stopped 2 to 3 days before starting treatment with Inhibace. If the fluid pills cannot be stopped, then the recommended starting dose of Inhibace is 0.5 mg by mouth once daily with close blood pressure monitoring. The dose may be adjusted based on response to the medication.
Congestive Heart Failure
The recommended starting dose for patients with congestive heart failure is 0.5 mg by mouth one time a day. The dose may be adjusted based on response to the medication.
The dose may also be adjusted in elderly patients and in patients with kidney problems or low sodium levels.
Ingredients
The active ingredient in Inhibace medication is cilazapril. The inactive ingredients include lactose monohydrate, cornstarch, hydroxypropyl methylcellulose, talc, sodium stearyl fumarate, and titanium dioxide. Iron oxide is used as a coloring agent.
Contraindications
Patients should avoid taking Inhibace if they:
- Are allergic to cilazapril or any of the other ingredients or excipients in Inhibace
- Have had angioedema (swelling underneath the skin) caused by an ACE-inhibitor or have hereditary or idiopathic angioedema
- Are unable to produce urine or produce only little amounts of urine
- Have swelling caused by buildup of fluid
- Are pregnant or plan to become pregnant
- Are breastfeeding or plan to breastfeed
- Take aliskiren and have diabetes or moderate to severe kidney problems
- Cannot tolerate galactose, cannot properly absorb glucose-galactose, or if they have Lapp lactase deficiency
Cautions
Serious Warnings and Precautions
The use of INHIBACE (cilazapril) is contraindicated during pregnancy (see CONTRAINDICATIONS). When used in pregnancy, angiotensin converting enzyme (ACE) inhibitors can cause injury or even death of the developing fetus. Pregnant women should be informed of the potential hazards to the fetus and must not take INHIBACE during pregnancy. Patients planning pregnancy should be changed to alternative antihypertensive treatments which have an established safety profile for use in pregnancy. When pregnancy is detected, INHIBACE should be discontinued as soon as possible and, if appropriate, alternative therapy should be started.

- Before Inhibace treatment is initiated, the following should be discussed with your doctor:
- All the medications you take, including prescription and nonprescription medications, vitamins, and herbal supplements
- Any allergies you may have
- Your medical history and current health conditions
- If you are pregnant, think you are pregnant, or have plans to become pregnant
- If you are breastfeeding or plan to breastfeed
- Angioedema can occur with Inhibace treatment. Black-skinned patients and patients who have had angioedema before have a greater risk. Seek medical care right away if you develop swelling, hives, or trouble breathing.
- Inhibace should be used with caution if you have obstructive cardiac problems such as mitral stenosis, aortic stenosis, and hypertrophic cardiomyopathy. Severe low blood pressure can occur.
- Low blood pressure can occur with Inhibace treatment. Monitor your blood pressure regularly.
- Taking Inhibace with aliskiren can increase the risk of low blood pressure, fainting, stroke, high potassium levels, and kidney problems in patients who have diabetes and/or moderate to severe kidney impairment.
- Some patients may develop a dry cough with Inhibace treatment. The cough generally disappears if the dose is lowered or Inhibace is stopped.
- Inhibace may increase the blood sugar lowering effects of oral diabetes medications or insulin. Patients with kidney problems are at greater risk. Blood sugar levels should be monitored as instructed by the doctor.
- Inhibace does not work as well in black-skinned patients of African descent.
- Treatment with Inhibace may lead to low counts of certain types of white blood cells, including neutrophils and granulocytes.
- Treatment with Inhibace can cause liver problems such as hepatitis, jaundice (yellowing of the skin or whites of the eyes), increased liver enzymes, or increased bilirubin levels.
- Treatment with Inhibace can cause high potassium levels, especially in patients with kidney problems and/or in patients taking potassium-sparing diuretics or medications that contain potassium.
- Treatment with Inhibace can cause anaphylactoid reactions when exposed to membranes such as during dialysis with high-flux membranes.
- Treatment with Inhibace can cause anaphylactoid reactions during low density lipoproteins (LDL) apheresis with dextran sulfate.
- Treatment with Inhibace can cause anaphylactoid reactions during desensitizing treatment with bee/wasp venom.
- Patients who receive injectable gold and are taking Inhibace are at risk for nitroid reactions that can cause facial flushing, nausea, vomiting, and symptomatic low blood pressure.
- Inhibace can cause low blood pressure during surgery or anesthesia.
- Treatment with Inhibace can cause kidney problems in some patients.
- Lactose monohydrate is contained in Inhibace tablets. Patients who cannot tolerate galactose, cannot properly absorb glucose-galactose, or have Lapp lactase deficiency should avoid taking Inhibace.
- Inhibace can cause harm to an unborn fetus; therefore, pregnant or breastfeeding women should avoid taking Inhibace.
Side Effects
The most common side effects that can occur with Inhibace treatment include headache, dizziness, and coughing; however, these are not all the side effects that can occur during treatment. Call your doctor right away if you develop side effects that are bothersome or continuous.
