Description
Fanapt (Iloperidone) is an atypical antipsychotic medication primarily used to treat schizophrenia. It works by targeting specific receptors in the brain, helping to balance the levels of neurotransmitters, such as dopamine and serotonin, which are involved in regulating mood and behavior. As a result, Iloperidone may alleviate symptoms like hallucinations, delusions, and social withdrawal in individuals with schizophrenia.

Directions
The initial dosing follows a specific titration method to avoid side effects like orthostatic hypotension. The medication is started at a low dose and gradually increased until the effective dose is reached. On the first day, an individual typically takes 1 mg orally twice a day, 12 hours apart. On the second day, the dosage is increased to 2 mg taken twice a day. From here, the dosage is increased daily by 2 mg increments up to the effective dose, which ranges from 12 mg to 24 mg taken orally twice a day.
The drug’s effectiveness for controlling symptoms may be delayed during the first 1 to 2 weeks of therapy. When taking Fanapt, be sure to follow the prescribed dosing schedule and consult a healthcare provider for any concerns or adjustments needed in the treatment plan.

Contraindications
Use caution and consult a healthcare professional before taking Fanapt. Avoid Fanapt if you have a known or suspected hypersensitivity of allergy to Fanapt or any ingredients in the formulation.
Cautions
- Before taking Fanapt, communicate the following to your healthcare provider:
- Any medications you are taking, especially those related to blood pressure, heart problems, or mood stabilizers.
- Any history of heart conditions, seizures, diabetes, or liver problems.
- Risk factors for low bone mineral density, such as family history or lifestyle habits.
- If you are pregnant, planning to become pregnant, or breastfeeding, as Fanapt may have potential risks for unborn babies and nursing infants.
- If you have a history of allergies or adverse reactions to medications.
- Increased instances of cerebrovascular adverse reactions like stroke or transient ischemic attacks have been reported in elderly patients. Consult your healthcare provider for alternative treatments if you fall into this category.
- Fanapt can prolong the QT interval, increasing the risk for arrhythmia and sudden death. Exercise caution and consider alternative antipsychotics first. If you are on medications known to prolong QTc or affect Fanapt metabolism, dosage changes and regular monitoring of potassium and magnesium may be required.
- Use Fanapt cautiously if you have a history of seizures or conditions that lower the seizure threshold. Your healthcare provider may need to modify your treatment accordingly.
- If you have a history of low white blood cell count or leukopenia/neutropenia, frequent complete blood count (CBC) monitoring is necessary during the first few months of Fanapt treatment.
- Cases of priapism, a prolonged and painful erection, have been reported. Immediate medical attention is required if this occurs.
- Fanapt may cause dizziness, tiredness, and low blood pressure. It is essential to use caution when driving or operating machinery until you know how the medication affects you.
- Fanapt can lead to a potentially life-threatening condition called Neuroleptic Malignant Syndrome (NMS). Be aware of NMS symptoms such as high fever, muscle stiffness, confusion, or rapid heartbeat, and seek immediate medical help if these symptoms develop.
- Fanapt may cause increased risk of suicidal thoughts or actions in some individuals, particularly younger adults. It is important to closely monitor your mental health while taking Fanapt and report any concerning changes to your healthcare provider.
- The medication may cause weight gain and elevated levels of blood sugar, lipids, and prolactin. Regular monitoring of these levels is advised during treatment with Fanapt.
- Fanapt may cause some individuals to experience involuntary movements, especially with long-term use. Consult your healthcare provider if this occurs.
Side Effects
Fanapt may cause side effects. Common side effects may include dizziness, drowsiness, dry mouth, stuffy nose, increased heart rate, weight gain, and low blood pressure. Patients may also experience other issues such as diarrhea, fatigue, and tremors. It is important to monitor these side effects and discuss any concerns with a healthcare professional.
In addition, Fanapt may cause more serious side effects that require immediate medical attention. Such side effects include seizures, dangerously low blood pressure, falls, problems with movement or thinking, a prolonged, painful erection, and an increased risk of death, suicide, stroke, and heart attack. This medication may also increase prolactin levels, which can cause side effects like females losing their period, production of breast milk, males losing their sex drive, or other related issues. If you’re taking Fanapt and experience any of these serious side effects, seek immediate medical advice.
