Description
Bicillin C-R (penicillin G benzathine and penicillin G procaine injectable suspension) is used to treat or prevent certain susceptible bacterial infections. It is given intramuscularly.
Bicillin C-R comes in a single-use syringe that is disposable.

Your doctor will determine an appropriate Bicillin C-R dosage for you. Your doctor or other healthcare provider will inject Bicillin C-R into the muscle of the buttocks or hip.
Ingredients
Penicillin G, penicillin benzathine, sodium citrate buffer; and as w/v, approximately 0.5% lecithin, 0.55% carboxymethylcellulose, 0.55% povidone, 0.1% methylparaben, and 0.01% propylparaben are contained in Bicillin C-R.
Contraindications
Bicillin C-R should not be administered to patients who are allergic to penicillin or to procaine.
Cautions
Bicillin C-R contains the following FDA Boxed Warning:
- Do not use intravenously or mix with other intravenous solutions. Inadvertent intravenous administration of Bicillin C-R has occurred, which has been associated with cardiorespiratory arrest and death.
- Before you begin receiving treatment with Bicillin C-R, tell your doctor about:
- All the prescription and over-the-counter medications you take
- Your allergies
- Your current health conditions and past medical history
- Your pregnancy or breastfeeding status
- Serious allergic reactions, including anaphylaxis, can occur with Bicillin C-R use. Patients who are allergic to penicillin or sensitive to multiple allergens are at an increased risk. If a patient experiences a severe allergic reaction, emergency treatment should be started immediately.
- Severe cutaneous reactions can occur with Bicillin C-R use. Patients should seek immediate medical care if any skin problems appear.
- Methemoglobinemia can occur with Bicillin C-R use, especially in patients with glucose-6-phosphate dehydrogenase deficiency, congenital or idiopathic methemoglobinemia, and cardiac or pulmonary compromise. The risk is also greater in infants under 6 months old and patients who are exposed to oxidizing agents or their metabolites while taking Bicillin C-R. If a patient’s skin appears blue, they should seek emergency care right away. Methemoglobinemia can result in seizures, coma, arrhythmias, and even death.
- Clostridium difficile (C. difficile) associated with diarrhea can occur with Bicillin C-R use. Patients should inform their doctor of any diarrhea that develops.
Side Effects
Hypersensitivity reactions, gastrointestinal conditions, anemia, abnormal blood cell counts, injection site reactions, sweating, blurred vision, and kidney problems are some side effects associated with Bicillin C-R. However, these are not the only side effects that can occur. Call your doctor right away if you have side effects that bother you or won’t go away.
